Income Eligibility Form
Even though all meals are free, families are asked to complete a Household Income Eligibility Form each school year. This form is required for federal reporting and helps determine school funding for programs that benefit all students. Completing the form does not affect meal access.
Forms are available at your school’s front office at the start of each school year.
